Information for all family members interested in counselling and/or group services must be entered fully to begin the process and all family members interested in participating must be present for the consultation session.

If you are interested in Group programs, please check our Groups section or call us to learn which groups we offer, and indicate on the application which one(s) you would like to participate in.

If you have an Employee Assistance Program (EAP), please call your EAP provider and do not complete this application.

If you require help completing the application, please call our office at 905-895-2371 and speak to a member of our Service Navigation Team.

You will be contacted to arrange a consultation session with a counsellor and/or our Service Navigation Team may recommend other services.  It is important that all family members that would like to participate in counselling are available during the scheduled consultation session.

During the consultation session, you and the counsellor will determine what services are most suitable for your situation.  The counsellor will also provide strategies and resources to use in the meantime.

If you are seeking services for a child under the age of 10, we will ask that only you attend the first session. If there is a custody arrangement, please bring Proof of Custody to the first session. Please note, if custody is shared, we will need consent from both parents prior to beginning services with a child.

The fee for the initial consultation appointment is $30, non-refundable. FSYR is able to provide exceptions to this fee, only in special circumstances, once proof of income is provided. No one is turned away from service because of inability to pay.

Payment is collected at the time of booking. 

The fee for ongoing services varies depending on the program selected.  Full fee for the Counselling Program is $120 per session.  If a reduced rate is required, this will be discussed at the Consultation Session and a fee will be determined (with proof of income). If you are 12 to 15 years of age and are seeking services without your parent’s knowledge, you may not be required to pay the fee.
